Case Summary
(Reason for referral):
Jose was referred by the LEP Committee/ Child Study police squad for Language Proficiency/Dominance appraisal in lead of academic difficulties.
Current level of performance:
Jose is currently repetition the 1st grade for the second time. He scored a quaternity-spot on the initial M-DOLPS-R and is entering the English for Speakers of Other Languages (ESOL) design on imposing 27, 2003. He has participated in the ESOL program for four semesters, for two hours daily. Jose also receives Home Language Arts (Spanish-S) program line for one hundred fifty minutes a week.
Assessment Results:
Jose was give the International Performance Scale to assess his intelligent functioning
The results of Joses performance profile reflect the sideline findings:
M-DCOLPS-R: ESOL level 3, well-disposed Language Inventory: Very limited-Independent,
Spanish: High Immediate, English: Low Intermediate.
Jose was also administered the Adaptive Behavior, Social History, Psycho-Educational, Vision, Hearing, Speech/Language, and Observation exams where he scored lower than normal. His IQ is 108 which places him the in the average range of intellectual functioning.

Goals & Benchmarks:
Academic:

-By August 2012 Jose will be able to recognize both baseborn and medium size words like dog, cat, house, and bird by sight with at least 50% accuracy.
-By December 2012 Jose will be able to have a grasp on small and medium words and will be able to start on hard words such as dictionary and follow.
Accommodations
Jose will be tested weekly with instructor made test.

Behavior:
Within the year Jose will be able to elaborate on his feelings and be able to run it verbally.
